Dear Jesus,
I realize that I am a sinner.  I know that you died on Calvary's cross for my sins, sickness, deceases, sorrows, and grief's.  Today I come to you on bended knees, and with a heart of repentance, I am asking you to forgiven me of all my sins, and wash me with your blood.  Cleanse me from all unrighteousness, and wash all of my sins away.  Come into my heart, and renew a right spirit within me.  I renounce satan, and accept you as my Lord and Savior.  As from today I am your child, and you are the Lord of my life.  I have turned my back on a life of sin, and death, and have taken up a life of eternity.  I thank you Lord for being my Savior!  Now Father, fill me with your precious Holy Spirit and empower me for your service.  All these things I ask in Jesus name!
